The Family Dental Plan emphasizes prevention, early detection and dental health maintenance, and was designed to provide dental coverage for our patients who do not have conventional dental insurance. As an enrollee you will save from 15% to 50% on all dental services we provide. Unlike conventional dental insurance plans that cover select services, The Highland Hills Family Dental Plan is exclusive to our patients and covers ALL dental services.

• Flouride Treatments
• Root Canals
• Dentures

BENEFITS TO YOU:
FIRST YEAR

• Once enrolled the plan begins immediately and will be in effect for 1 year.
• The first exam, routing cleaning, x-rays and flouride treatment will be covered at 100%
• Your next 6th month recall appointment will be covered at 50% off.
• All other services that we provide will be at 15% off.

CONSECUTIVE YEARS

• Once enrolled the plan begins immediately and will be in effect for 1 year.
• The first exam, routing cleaning, x-rays and flouride treatment will be covered at 100%
• Your next 6th month recall appointment will be covered at 50% off.
• All other services that we provide will be at 17% off.

ENROLLMENT FEES:
FIRST YEAR

• Individual 15 years and older $285.00
• Individual $285.00 plus $165.00 for each dependent (Spouses, children up to the age of 18 living at home and college students up to the age of 23)

CONSECUTIVE YEARS

• Individual 15 years and older $150.00.
• Dependent 14 years old and younger - no fee.
• Dependent will receive 17% off all dental care.
• Dependent has to have at least one parent enrolled in 1st inhouse plan and is currently enrolled in the 2nd plan.
• Consecutive year plan fees are subject to change with cost of living increases.
